Norman Bates is a movie character and murderer in "Psycho" (1960).
He suffers from a split personality. The overlaid typeface and narrowness of the letters represent the multiple personalities squeezing into and fighting inside one person. Through the high contrast of white and black (good and evil) and the extra thin paper the letters from the back shine through to the front and symbolise the underlying mental issues of the character.
