My Patient Visit is an online portal for patients to access their medical records and information pertaining to their care. The registration process involves two factor authentication and allows patients to use their account to connect to multiple care providers. The portal features Dr. Karen Foreya, an avatar used to guide users through the portal.

Once logged in, every patient will have a Dashboard where they can see a list of all of their upcoming appointments and pre-register for those appointments from the dashboard. Users are able to upload a photo to their profile, which also syncs back to the office. In the medical industry, notifications are important, so every user has a Health Feed with notifications of any activity to their MyPatientVisit account or medical record.

Users are able to update their demographic information and have it sync directly to their care provider's Practice Management System.

A unique feature on the MyPatientVisit portal is the Pre-Registration feature. This user experience is combines all of the "pages" or "forms" that a patient would fill out when they go in for a doctor's visit in one easy location. The user interface features a background image of a clipboard and pencil, something that patients are used to seeing when they are filling their forms out on paper, and also includes a progress bar to indicate where the user is within the pre-registration process.

During the pre-registration process, users are required to fill out all of the necessary forms needed for their visit. The screen below shows the users how many forms they will be required to fill out and the progress of each form.

Below is an example of an automated e-mail generated when their is a visit summary published to a patient's account. These e-mails are very important as interaction between patients and doctors make up for Meaningful Use requirements. Again, I am exploring gamification with using the avatar to engage patients in the e-mail.
