Chronic Kidney Disease Project
A Texas-wide initiative to address disparities in chronic kidney disease care in the African American and Hispanic communities. As part of this initiative we worked with primary care physicians, vascular surgeons and nephrologists to promote cooperation and education and with patients to increase awareness.
A 2-sided flyer distributed to patients.
A poster distributed in the target communities.
A kit for vascular surgeons to distribute to their patients explaining how to care for their fistula. The kit includes an exercise ball, a tourniquet and a brochure all contained inside a plastic bag.
A dual-launguage direct mail piece targeting people who take blood pressure medication and have diabetes.
A card with a detachable response card distributed to diabetes patients with chronic kidney disease. A silicone wristband with the message "Save Veins - No IV/Lab Draws" was also included.
