Manuals and Forms is a course taken during my final semester at Heritage College. It included a project titled simply 'Form Design'. The end goal for this project was to create a form that could be easily edited by clients of a fictional business.

Walsh Wedding Planning requires some standard fields such as names and contact information, but from then on all the fields are original. Different field formats were used for each of the categories: radio buttons, text fields, drop down lists, etc.

The original design was created in Adobe InDesign, setting up the basic field options. It was then completed and finalized in Adobe Acrobat and saved as an editable PDF that could be easily used by clients wishing to download the consultation form.

I included a 'clear' button for mistakes, a 'validate' button to confirm that important fields were filled out, and a 'print' button which does exactly as the name describes.
